By completing this form you are agreeing to the Season 14 dates and expectations and have been approved by your Curriculum Leader and/or Head of School to join Cohort 21 for the 2025–2026 academic year. You and your direct supervisor also understand that the cost of this professional development is $975, which will be billed to your school in October 2025 by CIS Ontario.

As this program has a capacity limit, registrations are not refundable.

Cancellation Policy
CIS Ontario reserves the right to cancel a program if there is insufficient enrollment. In this unlikely situation, participants will be notified in advance of the event and given a full refund, if a fee was charged. CIS Ontario, however, cannot be responsible for any additional financial obligations that the participant may incur because of the cancellation.

Please note, participant lists that include contact details will be shared with other participants of the event and this may include organizations/vendors if there is a sponsorship component.
